If you have questions after you have read through the information below, please contact your Major Faculty Advisor or the Humanities Advisement Center. We will be happy to help with any of your academic support questions.

Academic Support Issues

Students on Warning or Probation:

1. Fill out the forms
• Academic Improvement Plan
• Academic Success Workbook
• Academic Success Workbook Answer Sheet
2. Meet with your major faculty advisor (see the list below)
3. Return the forms to the Academic Support Office.

Students Returning from Suspension:

1. Fill out the forms
   • Returning Suspended Letter
   • Graduation plan for returning suspended students
2. Meet with the Supervisor of the Advisement Center (Mary Anne Rich) in 1175 JFSB to have a graduation plan approved
3. Meet with your major faculty advisor to have the forms reviewed and signed
4. Return the forms to the Academic Support Office in 2502 WSC.
